To expedite your check-in process and ensure we have your complete medical history at the time of your appointment, please fill out the new patient form, which you can download below. 

 

If you choose to complete the form on the day of your appointment, please arrive 10 minutes early. Additionally, please bring your current insurance card, Photo ID, a list of your current medications, and any prescription glasses or contacts that you may wear.
